How you’ll make an impact in this role - Guide patients and families through the entire continuum of care, serving as a dedicated navigator to ensure a seamless transition into and through the rehabilitation process. - Collaborate as a strategic liaison between medical staff, administration, and insurance providers to streamline communication and advocate for the patient’s treatment plan. - Analyze care outcomes and patient data to drive process improvements that directly enhance the quality of service provided by the clinical team. - Champion health equity by securing essential referrals and follow-up support for uninsured or underinsured patients, ensuring no one is left behind in their recovery journey. - Cultivate a high-performing team environment within the Patient Access Admitting department, occasionally traveling to local acute care facilities to facilitate patient transitions What minimum requirements you’ll need Licensure / Certification / Registration: - Licensed Registered Nurse credentialed from the Kansas Board of Nursing required. - BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: - Diploma from an accredited school/college of nursing OR required professional licensure at time of hire. What additional preferences we're seeking May require local travel to acute care facilities.
