How you’ll make an impact in this role Design Holistic Care Pathways: You will create comprehensive care plans that span the entire continuum, ensuring that a patient’s unique values and family preferences are at the heart of every clinical decision. Be a Patient Advocate: You’ll serve as the primary champion for your patients, proactively identifying and removing barriers to care and securing the essential resources they need to thrive. Master the Transition of Care: Acting as a vital liaison, you will coordinate between the hospital and post-acute services, ensuring that the hand-off is safe, clear, and supportive. Drive Clinical Excellence: You’ll use your expertise to guide medical record documentation and provide level-of-care recommendations that align treatment options with the best possible outcomes. Optimize Resources: You will advocate for the efficient and effective use of clinical resources, balancing high-quality care with smart operational stewardship. What minimum requirements you’ll need Licensure / Certification / Registration: Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date required. Case Manager credentialed from the American Case Management Association (ACMA) preferred. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
