We Are Hiring:
Launched in 2002, Ascension Living HOPE is
a local, comprehensive program of healthcare and support services
based on the Program of All-Inclusive Care for the Elderly
Monday - Friday - 8 hour shifts
What You Will Do:
8:00am - 4:30pm
Provide direct nursing care to patients in
the long term care setting in accordance with established policies,
procedures and protocols of the healthcare organization.
What You Will Need:
- Implement and monitor patient care plans.
- Monitor, record and communicate patient condition as
- Serve as a primary coordinator of all disciplines for
well-coordinated patient care.
- Note and carry out physician and nursing orders.
- Assure resident safety by identifying safety hazards and
emergency situations and initiates corrective action as
- Assess and coordinate patient's discharge planning needs with
members of the healthcare team.
Licensure / Certification /
- BLS Provider obtained within 1 Month (30 days) of hire date or
job transfer date required.
- Registered Nurse obtained prior to hire date or job transfer
- Diploma from an accredited school/college of nursing
